Apply for Recovery Supports Grant

On behalf of the Minnesota Department of Human Services (DHS), Proof Alliance is seeking proposals from applicants to provide comprehensive, community-based long-term recovery services to pregnant and parenting women with histories of alcohol use disorders and substance use disorders.

Grant Application Due July 7, 2022 by 5:00 pm CST

The grant period will begin as soon as the grant review and selection process is completed and will end on June 30, 2023. There is the possibility of extending contracts for at minimum another year at that time.

Step 3: Review Grant Contract Terms

Review the Grant Contract Terms before submitting the grant application. In the application, applicants will signify their acceptance or attach exceptions to the contract terms.

Step 4: Submit a full grant application

Applications are due on Thursday, July 7 by 5:00 p.m. Please submit your application via e-mail to alyssa.huether@proofalliance.org.

Step 5: Review of grant applications

A review process will be conducted by Proof Alliance and will include site visits and a review committee.

Step 6: Notification of recipients

Organizations that are approved to receive grant funding will be contacted by Proof Alliance staff. A grant agreement will be developed that details project dates, payment schedules, required grant reports, and other pertinent information. Organizations will be notified via letter if their project is not selected to receive funding.

  • Organization’s current year operating budget with most recent actual expenses compared (budget to actual)
  • Proof of tax-exempt status (if applicable)
  • Most recent year-end financial statements
  • Most recent tax return (example: Form 990)
